Referring a case for diagnosis

For an efficient and effective diagnostic service it is vital that a full clinical description, growth charts and eventually MR-Images of the patient are provided. Please use the Patient Information and Referral Form .

For cases with an uncertain diagnosis please fill out the same Referral Form and contact us by email or telephone for discussion of the case.
